• Transportation workers/drivers list, mentioning their years of experience in waste transportation
• Aerial map and clear photos of the parking area

Complete form related to each vehicle, including:

• Clear photos of each vehicle from all sides, with a hazardous waste label
• Clear photos of the ownership and insurance
• Clear copies of the drivers id cards and their driving licenses

• A valid invoice copy between the parties
• Any other attachments if the waste is dangerous

• A copy of a valid commercial register CR
• Clear photographs of the site (interior and exterior), taken from multiple angles, showing the surrounding activities relative to the work site

• A copy of the invoice or the shipping statement showing a detailed shipment content
• A copy of the technical information (technical data, catalog, pictures...) of the material(s) to be licensed
• The importer’s license and the import permit issued by the Supreme Council for Environment for the radiation emitting device or radioactive source mentioned in the invoice/shipping manifest
• End-user license and use permit issued by the Supreme Council for Environment for the radiation emitting device or radioactive source mentioned in the invoice/shipping manifest
