Replacement/Extract of Death Certificate

Legal inheritance document if no direct relationship exists
Official general or specific power of attorney for the death certificate
Proof of legitimate interest for in-person requests to issue the death certificate
ID of the deceased (ID card or passport)
ID of the person reporting the death (ID card or passport)
Completed form for a replacement/duplicate death certificate (if applying in-person)
The applicant must be a first-degree relative (provide proof of relationship)

Death Certificate Request

Hospital's original medical report detailing the cause of death
ID of the deceased (ID card or passport)
ID of the individual reporting the death (ID card or passport)
Filling out the Death Notification Form
Deaths must be reported within 72 hours as per Article (12) of the Deaths Law which requires reporting deaths in Bahrain within 72 hours and those outside the Kingdom within 60 days

Death Certificate Request

Fill out the death notification form
Identity of the deceased
Identity of the deceased’s family
Notification of cause of death / death certificate certified by the country where the death occurred (death outside Bahrain) for Bahrainis only

Replacement/Extract of Death Certificate

Fill out a request form to replace a lost death certificate
ID card(s) or passport(s) of the deceased’s family member(s), and a copy of the respective document
The deceased’s passport and copy, plus an ID card, if any, and a copy
For the authorized person: Proof of authorization with an ID card, plus an additional copy

Death Certificate Request

0.4 BD , The certificate issuance fee if notification is made after 72 hours and up to three months from the date of death
0.8 BD , The fee for issuing a death certificate if the notification is made after 3 months and up to twelve months from the date of death
Free , For first 72 hours

Replacement/Extract of Death Certificate

1 BD , A fee of one dinar is required to obtain a copy of the birth or death certificate if the submitted application states the date of birth/death in terms of the day, month and year, and the fee is doubled if the application solely states the year of the occurrence
