Passport Lost/Damaged Replacement Issuance Request

The Nationality, Passports & Residence Affairs provides for citizens the ability to submit a request to issue a replacement for the lost or damaged passport.

Replace a lost or damaged passport

  • Applicant (or an authorized person) must be present when collecting the passport for an incompetent person at the Isa Town branch
  • Police report of the incident from the relevant police station (replacement of a lost passport)

In the event of a lost passport

  • Certificate issued by the General Department of Investigation and Forensic Evidence
  • A recent and clear 4×6 personal photo (coloured) with a white background. The photo must be attached separately in JPEG format. Click here to view the requirements for a passport photo

In the event of a damaged passport

  • A letter stating the reason for the passport’s damage
  • Bring the damaged passport, if available
