To request for the issuance of a new death certificate or to request for a duplicate copy.

Death Certificate Request

  • The original medical notification of the cause of death and must be stamped by the authority where the death occurred


Submit a Request for Reissuing a Lost Death Certificate

  • If the applicant is personally attending, they must be a first-degree relative


Death Certificate Request

  • Fill out the death notification form
  • Identity of the deceased
  • Identity of the deceased’s family
  • Notification of cause of death / death certificate certified by the country where the death occurred (death outside Bahrain) for Bahrainis only


Submit a Request for Reissuing a Lost Death Certificate

  • Fill out a request form to replace a lost death certificate
  • ID card(s) or passport(s) of the deceased’s family member(s), and a copy of the respective document
  • The deceased’s passport and copy, plus an ID card, if any, and a copy
  • For the authorized person: Proof of authorization with an ID card, plus an additional copy


Submit a Request for Reissuing a Lost Death Certificate

  • 1 BD , A fee of one dinar is required to obtain a copy of the birth or death certificate if the submitted application states the date of birth/death in terms of the day, month and year, and the fee is doubled if the application solely states the year of the occurrence


Death Certificate Request

  • 0.4 BD , The certificate issuance fee if notification is made after 72 hours and up to three months from the date of death
  • 0.8 BD , The fee for issuing a death certificate if the notification is made after 3 months and up to twelve months from the date of death
  • Free , For first 72 hours
